West Gables Introduces Comprehensive Chronic Illness Support Care Model

Miami’s West Gables Health Care Center has launched a new comprehensive Chronic Illness Support initiative in collaboration with AccentCare, a palliative care specialist. The program integrates palliative care, pastoral support, and various services with the facility’s specialized clinical programs. The center’s patient-centric approach, developed with the support of Marquis Health Consulting Services, focuses on both physical and spiritual wellness for chronically ill patients. The palliative care component involves a trained team offering emotional and spiritual support to enhance the quality of life for patients and their families. The initiative also supports Chronic Kidney Disease Management, Neuro Recovery, Pulmonary Rehabilitation, education, health coaching, coordinated care, and enrichment activities. West Gables Health Care Center holds a five-star rating and collaborates closely with experienced physicians from West Gables Rehab Hospital.
